NHS Managed Agents
A strategic analysis of how AI-powered managed agents could transform the administrative structure of a merging NHS Integrated Care Board — reducing costs, accelerating response times, and freeing clinical and professional staff to focus on what only people can do.
Overview
Rethinking how an NHS organisation works.
When three NHS Integrated Care Boards — NHS BSW, NHS Somerset, and NHS Dorset — merged into a new South West Cluster ICB, it created one of the most significant NHS restructuring challenges in recent years. The new organisation would cover six geographic place areas, employ hundreds of people across five directorates, and manage a combined commissioning budget of around £8 billion.
The question put to us was deceptively simple: given the technology that now exists, and given the financial pressures NHS ICBs are operating under, is there a smarter way to design this?
We used AI tools to process all 344 proposed job descriptions across the merged structure — extracting key functions, identifying duplication, and mapping which tasks follow predictable rules versus which genuinely require professional judgment. The result was a structured case for a network of 24 AI managed agents, each with a defined role, a human supervisor, and strict escalation rules.
The analysis did not stop at strategy. We built a working implementation — a complete FOI Agent, deployable on NHS infrastructure — to demonstrate that this is not speculative. The architecture exists. The compliance framework exists. The question is how to deploy it safely.
What We Explored
Three lines of inquiry.
The Strategic Case
Using AI to analyse 344 job descriptions, map duplicated functions, and build a case for what an agent-augmented NHS organisation could look like — and what it would mean for patients, partners, and professional staff.
The Technical Architecture
How managed agents are actually built — the agent definition file, N8N workflow orchestration, the Anthropic API, data sovereignty options for NHS systems, and the compliance framework that keeps humans in control.
A Working Implementation
A complete, deployable FOI Agent — real workflow files, real knowledge base, real agent definition — built around a fictional NHS ICB so any organisation can adapt it by replacing three files with their own data.
Live Demo
Submit a real FOI request to the agent.
