Overview

Rethinking how an NHS organisation works.

When three NHS Integrated Care Boards — NHS BSW, NHS Somerset, and NHS Dorset — merged into a new South West Cluster ICB, it created one of the most significant NHS restructuring challenges in recent years. The new organisation would cover six geographic place areas, employ hundreds of people across five directorates, and manage a combined commissioning budget of around £8 billion.

The question put to us was deceptively simple: given the technology that now exists, and given the financial pressures NHS ICBs are operating under, is there a smarter way to design this?

We used AI tools to process all 344 proposed job descriptions across the merged structure — extracting key functions, identifying duplication, and mapping which tasks follow predictable rules versus which genuinely require professional judgment. The result was a structured case for a network of 24 AI managed agents, each with a defined role, a human supervisor, and strict escalation rules.

The analysis did not stop at strategy. We built a working implementation — a complete FOI Agent, deployable on NHS infrastructure — to demonstrate that this is not speculative. The architecture exists. The compliance framework exists. The question is how to deploy it safely.
